Step I

Register for a School Tour

  • Tour at our Early Childhood Campus for children born in 2018 and 2019

  • Tour at our Elementary and Middle school for children born in 2017 and before


  • Wednesday, January 19th | 11:00 AM - 12:00 PM

  • Tuesday, February 8th | 12:00 PM - 1:00 PM

Step II

Apply for Entry and Schedule an Assessment

Complete an application form (FORM HERE) and submit electronically to or by mail/in person at your child's School Campus OR the Main Office, 29 Mott Street, NYC 10013. You may also complete this after your tour.

PreK students will be invited for a group assessment date in March. K-8th grade students will be invited to schedule an assessment day after the school tour. If you cannot schedule at that time, contact or call 212-962-5265 to schedule your assessment. A $50 assessment fee (non-refundable) per student payable to Transfiguration School

Step III

Register and Submit Documentation

Once you have received official notification of acceptance, you must complete the necessary forms for registration. You must also submit copies of the following records and required payment within two weeks of the date on your acceptance letter or your placement into the school will be forfeited. You may submit these forms in person or email scanned copies to

1) Birth Certificate

2) Health Examination Form

3) Enrollment Agreement

4) Optional: Baptism Certificate if Catholic

  • Transfiguration School uses SMART TUITION to collect its payments. Instructions on how to make your payments electronically will be issued at registration.
